Provides information to individuals who are presently in facilities, or are in immediate risk of entering State Institutions or Nursing Facilities, about community options. Upon receiving requests from consumers and / or families to assist in getting them out of facilities.

For individuals who want to move out of institutions, CILSWKS provides assistance in finding housing, locating needed services, and other needs as requested by the individual and / or family. Once the individual is out of the facility, all other services become available.
